    Background:

    • Aneurysmal coronary artery disease (ACAD) is a rare condition.
    • Its distinct clinical characteristics and natural history remain incompletely understood.

    Purpose of the Study:

    • To investigate the clinical and historical features of ACAD.
    • To determine the natural history and long-term outcomes of ACAD.

    Main Methods:

    • Retrospective review of registry data from the Coronary Artery Surgery Study (CASS).
    • Identification and analysis of 978 patients with ACAD.
    • Comparison of clinical features and outcomes between patients with and without ACAD.

    Main Results:

    • No significant differences were observed in risk factors (hypertension, diabetes, lipid abnormalities, smoking) between ACAD and non-ACAD patients.
    • Incidence of myocardial infarction, angina severity, and peripheral vascular disease showed no significant variation between groups.
    • Five-year medical survival rates were comparable for both patient groups.

    Conclusions:

    • Aneurysmal coronary artery disease is not a distinct clinical entity.
    • ACAD appears to be a variant presentation of coronary atherosclerosis.
    • Further research may elucidate specific pathophysiological mechanisms.
