    Summary

    Acetaminophen is safe for individuals with stable chronic liver disease at recommended doses. This study found no evidence of liver damage or drug accumulation in patients taking acetaminophen.

    Background:

    • Chronic liver disease affects drug metabolism and safety profiles.
    • Acetaminophen (APAP) is a common analgesic and antipyretic.
    • Evaluating APAP safety in liver disease is crucial.

    Observation:

    • Six subjects with stable chronic liver disease received 4.0 gm of APAP daily for 5 days.
    • Mean acetaminophen half-life (t 1/2) was 3.42 hr, with no drug accumulation or hepatotoxicity observed.
    • A double-blind, crossover study in 20 subjects confirmed APAP's tolerability at 4.0 gm daily for 13 days.

    Findings:

    • Acetaminophen was well tolerated in subjects with stable chronic liver disease.
    • One subject experienced transient symptoms and lab abnormalities, later determined to be unrelated to APAP.
    • No adverse reactions or hepatotoxicity were indicated during the study.

    Implications:

    • Therapeutic doses of acetaminophen are safe for patients with stable chronic liver disease.
    • No contraindications exist for using acetaminophen in this patient population.
    • This supports the continued use of acetaminophen for pain and fever management in liver disease.
