    Background:

    • The action spectrum for eliciting a reaction was identified between 400-500 nm (blue-violet light).

    • Simultaneous or alternating exposure to blue-violet and red light significantly inhibited weal formation.
    • Local antihistamine injections did not block the urticarial reaction.
    • Histamine depletion using polymyxin B sulfate did not prevent the reaction, suggesting histamine's limited role in this patient's weal production.

    Implications:

    • Visible light therapy, specifically using red light, may offer a novel treatment approach for solar urticaria.
    • These findings challenge the traditional understanding of histamine's central role in solar urticaria pathogenesis.
    • Further research is warranted to elucidate the precise mechanisms underlying histamine-independent weal formation in this condition.
