    Background:

    • Sudden cardiac death (SCD) during physical exertion is a significant concern.

    Purpose of the Study:

    • To investigate the causes and circumstances of sudden cardiac death during exertion.
    • To analyze the role of underlying cardiac diseases in exertion-related SCD.

    Main Methods:

    • Studied 226 cases of sudden death during sports and 1,729 autopsy cases.
    • Performed exercise tests on 138 patients with various cardiac diseases.

    Main Results:

    • Marathon running was the most frequent sport associated with SCD (52.7%).
    • Coronary arteriosclerosis (67.0%) was the primary autopsy finding in exertion-related SCD.
    • Underlying conditions like cardiac hypertrophy and valvular heart disease also contributed.
    • SCD risk increased with exertion in patients with coronary arteriosclerosis or cardiac hypertrophy.
    • Exercise tests revealed potential abnormalities in patients with arrhythmia or hypertrophic cardiomyopathy post-exercise.

    Conclusions:

    • Predisposing heart disease is a key factor in sudden cardiac death during exertion.
    • Severe heat and overwork can precipitate SCD.
    • Vigilance is essential, particularly at the end of endurance events.
