[Case report of large endometrial stromal sarcoma]

Background:

• A 56-year-old woman presented with atypical genital bleeding, abdominal pain, and large pelvic tumors.
• Initial laboratory tests were noncontributory, necessitating further investigation.

Observation:

• Surgical exploration revealed a massive tumor (30 x 11 x 8 cm, 1600g) with polypoid growth and significant necrosis.
• Histopathological examination showed tumor cells with endometrial cell morphology and a high mitotic rate (20 figures/10 HPF).

Findings:

• The patient underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy.
• The large, aggressive tumor was successfully excised.

Implications:

• The case highlights a rare, large uterine tumor with aggressive features.
• Postoperative management with polysaccharide-K (PSK) appeared effective in ensuring the patient's recovery.
