Pyoderma gangrenosum. Response to cyclophosphamide therapy.

Summary

Severe pyoderma gangrenosum can be effectively treated with cyclophosphamide, even without associated systemic disease. This immunosuppressive therapy offers a new option for patients unresponsive to other treatments.

Background:

Pyoderma gangrenosum (PG) is a rare, inflammatory neutrophilic dermatosis.
Often associated with systemic diseases, PG can be challenging to manage.

Observation:

A 46-year-old woman presented with a 17-year history of severe, treatment-resistant pyoderma gangrenosum.
Her condition was refractory to systemic corticosteroids and sulfones.

Findings:

Complete remission of pyoderma gangrenosum was achieved with 150 mg/day of cyclophosphamide.
Cyclophosphamide demonstrated significant efficacy in this recalcitrant case.

Implications:

Immunosuppressive therapy, specifically cyclophosphamide, should be considered for severe, refractory pyoderma gangrenosum.
Treatment decisions for pyoderma gangrenosum should not be limited by the absence of associated systemic disease.
