    A woman with a history of hemipelvectomy successfully delivered a healthy baby via vertex presentation. External uterine support during pregnancy may improve fetal presentation in similar cases.

    Background:

    • Hemipelvectomy is a radical surgical procedure involving the removal of half of the pelvis.
    • Pregnancy following hemipelvectomy presents unique challenges for both the mother and fetus.
    • Optimal fetal positioning for vaginal delivery can be compromised by pelvic alterations.

    Observation:

    • A 32-year-old woman, 14 years post-hemipelvectomy, achieved a normal vertex presentation delivery.
    • During pregnancy, she utilized forearm crutches for ambulation and a custom sling for uterine support.
    • The patient reported comfort and perceived support from the external sling.

    Findings:

    • The patient delivered a healthy male infant via vertex presentation.
    • The use of an external uterine support sling during pregnancy was noted.
    • This case suggests a potential benefit of external uterine support in achieving favorable fetal presentation after hemipelvectomy.

    Implications:

    • External uterine support may be a viable strategy to improve fetal presentation in pregnant individuals with pelvic deformities.
    • This approach could potentially increase the likelihood of successful vaginal delivery in high-risk pregnancies.
    • Further research is warranted to explore the efficacy and mechanisms of external uterine support in post-hemipelvectomy pregnancies.
