Myelopathy after herpes zoster.

    Summary

    A rare progressive myelopathy following herpes zoster infection was observed. Antiviral treatment with vidarabine halted disease progression but did not restore neurological function.

    Background:

    • Herpes zoster (shingles) can cause neurological complications.
    • Progressive myelopathy following herpes zoster is an uncommon but serious condition.

    Observation:

    • A 73-year-old woman with a history of ovarian cancer developed herpes zoster affecting the lumbosacral region.
    • She subsequently experienced a progressive myelopathy characterized by normal myelography and cerebrospinal fluid (CSF) pleocytosis.

    Findings:

    • The patient received ten days of vidarabine (15 mg/kg/day) for antiviral therapy.
    • Vidarabine treatment halted the progression of the myelopathy.
    • No recovery of neurological function was observed after treatment.

    Implications:

    • Direct viral invasion of the spinal cord is the suspected cause of this rare myelopathy.
    • Antiviral therapy may be beneficial in arresting the progression of post-herpetic myelopathy.
    • Further research is needed to explore treatment strategies for functional recovery.
