    Summary

    High-water-content hydrogel implants show good tolerance in rabbit corneas, altering refractive state. In a human patient, the implant reduced corneal edema without refractive changes, indicating potential therapeutic applications.

    Background:

    • Previous studies demonstrated hydroxyethyl methacrylate (Permalens) hydrogel implants alter rabbit eye refractive state.
    • Investigated the biocompatibility and effects of intrastromal hydrogel implants in a human cornea with aphakic bullous keratopathy.

    Observation:

    • A human cornea with an intrastromal hydrogel implant was examined post-penetrating keratoplasty using light and transmission electron microscopy.
    • Histological analysis revealed a lack of edema in the epithelium and anterior stroma, while the posterior stroma remained edematous.

    Findings:

    • The hydrogel implant was well-tolerated for six months, reducing epithelial edema.
    • Minimal fibroblastic activity and no signs of inflammation, ulceration, or neovascularization were observed at the implant-stroma interface.
    • The intrastromal implant did not alter the refractive state in the human patient.

    Implications:

    • Intrastromal hydrogel implants demonstrate good biocompatibility and potential for managing corneal edema.
    • Further research may explore optimizing hydrogel properties for refractive correction and therapeutic benefits in corneal diseases.
