    Summary

Epilepsy can be the sole symptom of cerebral arteriovenous malformations (AVMs). Advanced CT scanning can detect AVMs missed by traditional angiography, aiding diagnosis.

    Area of Science:

    • Neurology
    • Neuroradiology
    • Neurosurgery

    Background:

    • Epilepsy is a common neurological disorder.
    • Cerebral arteriovenous malformations (AVMs) are complex vascular abnormalities in the brain.
    • AVMs can present with various neurological symptoms, including seizures.

    Purpose of the Study:

    • To investigate epilepsy as the sole manifestation of cerebral AVMs.
    • To evaluate the diagnostic utility of CT scanning in detecting AVMs.
    • To analyze the clinical and radiological characteristics of AVMs presenting with epilepsy.

    Main Methods:

    • Retrospective analysis of 27 patients with epilepsy and cerebral AVMs.
    • Review of clinical data, EEG findings, and imaging studies (angiography, CT).
    • Comparison of CT findings with conventional angiography.

    Main Results:

    • Epilepsy was the only presenting symptom in all 27 patients.
    • AVMs varied in size and location, with frontal lobe involvement being most common.
    • CT scanning detected vascular enhancement in AVMs, including some missed by angiography.
    • Infusion CT successfully identified two angiographically occult AVMs.

    Conclusions:

    • Epilepsy can be the exclusive clinical manifestation of cerebral AVMs.
    • CT scanning, particularly infusion CT, is a valuable tool for diagnosing AVMs, especially when angiography is inconclusive.
    • Early and accurate diagnosis of AVMs is crucial for appropriate management and prevention of complications.
