Phenytoin-induced lymphadenopathy.

T L Schwinghammer, D L Howrie

    Drug Intelligence & Clinical Pharmacy
    |June 1, 1983
    PubMed
    Summary

Phenytoin therapy can cause various lymph node abnormalities, including benign hyperplasia that mimics malignant lymphoma. Careful evaluation is crucial to distinguish drug-induced lymphadenopathy from true lymphoma for appropriate patient management.

    Area of Science:

    • Pharmacology
    • Oncology
    • Pathology

    Background:

    • Phenytoin is an anticonvulsant medication with known associations to lymph node abnormalities.
    • These abnormalities range from benign reactive changes to potentially fatal malignant lymphoma.

    Observation:

    • A patient on chronic phenytoin therapy presented with enlarged inguinal lymph nodes.
    • Initial diagnosis suggested malignant lymphoma, leading to hospitalizations and cytotoxic treatment.

    Findings:

    • Repeat biopsy and node reexamination revealed phenytoin-associated hyperplasia, not malignant lymphoma.
    • Phenytoin-induced lymphadenopathy can present similarly to malignant lymphoma.

    Implications:

    • Misdiagnosis of phenytoin-associated lymphadenopathy can lead to unnecessary and harmful treatments.
    • Thorough evaluation is essential for patients on phenytoin presenting with lymphadenopathy to differentiate benign drug reactions from malignancy.
