    Summary

    Wegener granulomatosis can present as a rare cranial mass. High-resolution CT imaging revealed its extension into the cavernous sinus, causing internal carotid artery occlusion.

    • The mass originated in the infratemporal fossa and extended through basal foramina into the cavernous sinus.

    • The cranial mass caused complete occlusion of the internal carotid artery.
    • HRCT showed no bone destruction, with a normal parotid gland and nasopharynx.

    Implications:

    • Wegener granulomatosis should be considered in the differential diagnosis of parapharyngeal masses with intracranial extension.
