    Background:

    • Acute disseminated encephalomyelitis (ADE) is an inflammatory neurological disorder.
    • Understanding the neuroimaging features of ADE is crucial for diagnosis and management.

    Observation:

    • Computed tomographic (CT) scans were performed on eleven patients diagnosed with ADE.
    • Four patients exhibited normal CT scans despite multiple examinations.

    Findings:

    • Seven patients showed abnormalities on CT scans, including cortical-enhancing lesions, deep white matter and basal ganglia low-density lesions, and brainstem edema.
    • A common observation was a delay between clinical symptom onset and CT lesion detection.
    • Clinical recovery correlated with improvements in CT scan abnormalities.

    Implications:

    • CT findings support the involvement of both vascular injury and demyelination in ADE pathogenesis.
    • The study highlights the dynamic nature of CT findings in ADE, correlating with clinical course.
    • Limited correlation between clinical course and specific CT lesion characteristics was noted.
