    Background:

    • Lumbar disk disease can present with debilitating symptoms requiring intervention.
    • Non-surgical management is often the first line of treatment for lumbar disk disease.
    • Recurrence of symptoms necessitates re-evaluation and potentially different treatment strategies.

    Observation:

    • A 56-year-old woman initially responded well to a non-surgical program for lumbar disk disease.
    • Fourteen months post-initial treatment, she experienced a recurrence of her previous symptoms and findings.
    • Diagnostic imaging, including myelography, revealed a spinal block at T12-L1 and a herniated disk at L4-L5.

    Findings:

    • A neurilemoma, a type of nerve sheath tumor, was identified as the underlying cause.
    • Surgical removal of the neurilemoma was performed.
    • The patient experienced a complete recovery and remained well for 4 years post-surgery.

    Implications:

    • This case highlights the importance of considering alternative diagnoses when symptoms of lumbar disk disease recur.
    • Neurilemomas, though rare, can mimic or coexist with degenerative spinal conditions.
    • Surgical intervention for spinal tumors like neurilemomas can lead to excellent long-term outcomes.
