    Background:

    Purpose of the Study:

    • To analyze the 14-year experience of a Chest Unit with spontaneous pneumothorax.
    • To evaluate treatment outcomes, recurrence rates, and the role of surgery.

    Main Methods:

    • Retrospective analysis of 236 patients with 315 episodes of spontaneous pneumothorax over 14 years.
    • Review of treatment modalities including observation, intercostal tube drainage, and surgical interventions (pleural abrasion/pleurectomy).

    Main Results:

    • 95% of cases were initially treated successfully with observation or tube drainage.
    • Treatment choice did not impact recurrence rates; overall recurrence was approximately 20%.
    • Surgical intervention (pleural abrasion/pleurectomy) resulted in no recurrences; 40% incidence of third episodes without surgery after recurrence.

    Conclusions:

    • The majority of spontaneous pneumothorax cases can be managed non-surgically with good outcomes.
    • Surgical intervention is recommended for recurrent pneumothorax or contralateral occurrences.
    • No mortality was observed in the study cohort.
