    Background:

    • Reticulum cell sarcoma, a type of non-Hodgkin lymphoma, can present with diverse systemic manifestations.
    • Gastrointestinal lymphomas may rarely lead to paraneoplastic ocular complications.

    Observation:

    • A 55-year-old woman with known stomach reticulum cell sarcoma developed sudden vision decrease, anemia, and thrombocytopenia.
    • Ophthalmic examination revealed superficial retinal hemorrhages in both eyes.
    • A significant macular mass, mimicking a neoplasm, developed in the right eye.

    Findings:

    • The retinal hemorrhages gradually resolved.
    • The macular mass spontaneously disappeared within weeks.
    • Minimal pigment epithelium changes remained, with excellent visual acuity recovery.

    Implications:

    • This case highlights the potential for transient, neoplastic-simulating ocular masses in patients with hematologic malignancies.
    • Prompt recognition and monitoring are crucial for managing such paraneoplastic ocular events.
    • The resolution of ocular findings suggests a possible inflammatory or reactive process secondary to the underlying sarcoma.
