    Background:

    • Polycythemia rubra vera (PRV) is a rare myeloproliferative neoplasm.
    • Pregnancy in women with PRV presents unique challenges.
    • Limited data exists on maternal and fetal outcomes in PRV pregnancies.

    Observation:

    • A review of 13 pregnancies in 8 women with PRV was conducted.
    • Maternal outcomes were generally favorable, with a noted association with pregnancy-induced hypertension.
    • Perinatal outcomes were frequently poor, marked by abortion, preterm delivery, and stillbirth.

    Findings:

    • PRV complicates pregnancy, posing significant risks to fetal development and survival.
    • Pregnancy-induced hypertension appears to be a notable maternal complication.
    • The etiology, symptoms, and diagnostic criteria for PRV in pregnancy are discussed.

    Implications:

    • Case presentation of a young woman with successful management of two PRV pregnancies.
    • Proposed guidelines for hematologic monitoring and management during pregnancy and puerperium.
    • Highlights the need for specialized care and further research in managing PRV during gestation.
