    Background:

    • Urinoma, or pararenal pseudocyst, is a collection of urine that forms outside the renal pelvis or bladder.
    • It is most commonly caused by trauma, but can also result from spontaneous rupture of the collecting system or iatrogenic injury.

    Observation:

    • The patient presented with symptoms and signs suggestive of pyelonephritis, including flank pain, fever, and elevated white blood cell count.
    • However, urine cultures were negative, and further investigations were required to determine the underlying cause.

    Findings:

    • Advanced imaging, such as CT or MRI, revealed a urinoma.
    • The urinoma was successfully treated with drainage and management of the underlying cause.

    Implications:

    • This case underscores the importance of considering urinoma in the differential diagnosis of patients presenting with pyelonephritis-like symptoms, especially when bacteriologic confirmation is absent.
    • Prompt diagnosis and appropriate management are crucial for favorable patient outcomes.
