    Background:

    • Radial keratotomy (RK) is a refractive surgical procedure to correct myopia.
    • Complications of RK can include irregular astigmatism, hyperopic shift, and, rarely, late-onset cataract.

    Observation:

    • A 31-year-old male developed an intumescent cataract 16 weeks post-RK.
    • Corneal perforation occurred during the initial RK, though no direct lens capsule injury was noted.
    • Visual acuity declined to hand motions over four months.

    Findings:

    • Extracapsular cataract extraction was performed seven months after RK.
    • Post-operative correction of aphakia with a soft contact lens achieved a visual acuity of 6/6.

    Implications:

    • This case highlights a rare but significant delayed complication of radial keratotomy.
    • Intumescent cataract formation following RK necessitates careful monitoring and timely surgical intervention.
    • Successful cataract extraction and aphakic correction can restore excellent visual function in affected patients.
