    Summary

    Right hemisphere speech dominance is rare without early left hemisphere damage. Lateral preference patterns, particularly handedness and footedness, can predict cerebral speech dominance in neurological patients.

    Background:

    • Sodium Amytal speech testing is a method to determine cerebral speech dominance.
    • Lateral preference, including hand, foot, eye, and ear, has been studied in relation to speech representation.
    • Early-onset cerebral damage can impact speech lateralization.

    Purpose of the Study:

    • To investigate the relationship between lateral preference patterns and cerebral speech dominance.
    • To examine the influence of early-onset cerebral damage on speech representation.
    • To assess the predictive value of different lateral preference types for speech dominance.

    Main Methods:

    • Sodium Amytal speech testing was performed on 73 patients.
    • Four types of lateral preference (hand, foot, eye, ear) were assessed.
    • Patient history regarding age of onset and location of cerebral damage was considered.

    Main Results:

    • Most patients (63/73) exhibited left hemisphere speech dominance.
    • Right hemisphere speech dominance was rare and typically associated with early left hemisphere injury.
    • Left-sided motor and sense organ preferences were linked to right hemisphere speech dominance in cases with early left hemisphere injury.
    • Handedness and footedness showed relevance in predicting cerebral speech dominance in the general population.
    • Eye and ear preference had limited predictive value.

    Conclusions:

    • Right hemisphere speech dominance is uncommon without early left hemisphere lesions.
    • Lateral preference patterns, especially handedness and footedness, are valuable indicators of cerebral speech dominance.
    • Congruency of lateral preferences may offer insights into speech lateralization in neurological populations.
