    Background:

    • Ulnar deviation and metacarpophalangeal joint subluxation are common in rheumatoid arthritis.

    Observation:

    • Three elderly female patients presented with severe ulnar deviation and metacarpophalangeal joint subluxation.
    • All patients had interphalangeal joint osteoarthritis but no other rheumatic conditions.
    • Clinical presentation initially suggested rheumatoid arthritis in all three cases.

    Findings:

    • The patients' rheumatoid-like hand deformities were attributed to underlying osteoarthritis, not rheumatoid arthritis.
    • One patient developed septic arthritis, potentially linked to corticosteroid use.
    • Rheumatoid-like deformities appear to be more prevalent in the elderly population.

    Implications:

    • Clinicians should consider osteoarthritis as a cause of rheumatoid-like hand deformities in elderly patients.
    • Accurate diagnosis is essential to avoid misdiagnosis of rheumatoid arthritis and ensure appropriate management.
