    Progressive external ophthalmoplegia (PEO) can present with peripheral neuropathy, indicating a rare multisystem disease. Pathological findings reveal nerve fiber loss and unique cellular structures in Schwann cells.

    Background:

    • Progressive external ophthalmoplegia (PEO) is a rare neuromuscular disorder affecting eye movements.
    • Multisystem involvement can occur in PEO, but peripheral neuropathy is an uncommon manifestation.

    Observation:

    • A case study detailing PEO with associated peripheral neuropathy and other neural/visceral symptoms.
    • Detailed pathological examination of sural nerve biopsies was performed.

    Findings:

    • Sural nerve pathology showed significant loss of large myelinated fibers.
    • Evidence of segmental demyelination with remyelination was observed.
    • Electron microscopy revealed an excess of zebra bodies within Schwann cells.
