Multiple coronary artery dissections in old age. A unique case.

    Summary

    A rare case of dissecting aneurysms in the small coronary arteries was observed in a 70-year-old man with alcoholic cirrhosis and emphysema. This unique condition may have been caused by iatrogenic trauma on weakened vessels.

    Background:

    • Coronary artery dissections are rare, particularly involving distal branches.
    • Medial weakness predisposes vessels to dissection.

    Observation:

    • A 70-year-old male patient presented with alcoholic cirrhosis and pulmonary emphysema.
    • Multiple independent dissecting aneurysms were found in the distal epicardial branches of both coronary arteries.
    • Proximal coronary artery trunks were unaffected.

    Findings:

    • Dissections originated from vascular branch points and were associated with acute and chronic medionecrosis.
    • The findings suggest a unique presentation of coronary artery dissection.

    Implications:

    • Preexisting medial weakness, potentially exacerbated by iatrogenic trauma, might explain this rare condition.
    • This case highlights the importance of considering rare vascular pathologies in complex clinical scenarios.
