    Summary

Chronic primary intestinal pseudo-obstruction (CPIP) is a rare, often fatal condition mimicking bowel obstruction. Its cause remains unclear, though primary visceral neuropathy is suspected, with limited treatment success.

    Area of Science:

    • Gastroenterology
    • Neurology
    • Pathology

    Background:

    • Chronic primary intestinal pseudo-obstruction (CPIP) is an infrequent gastrointestinal motility disorder.
    • Its etiology is poorly understood, prompting investigation into potential underlying causes.

    Observation:

    • A review of 30 CPIP case reports was conducted to evaluate clinicopathologic findings.
    • Presenting symptoms and radiological findings were nonspecific in most cases.
    • Esophageal motility abnormalities were noted in 12 of 14 reported instances.

    Findings:

    • Intestinal histopathology frequently showed normal muscle, mucosa, and ganglion cells (over 50% of cases).
    • Clinical improvement was observed in less than half of the patients (48%).
    • A significant mortality rate of 30% was documented among the reviewed cases.

    Implications:

    • CPIP presents as a complex and often fatal condition, mimicking mechanical bowel obstruction.
    • Primary neurologic or muscular disease is a potential explanation, but requires further definitive documentation.
    • Further research is needed to elucidate the exact etiology and improve management strategies for CPIP.
