    Summary

Hospital-acquired myiasis is a rare nosocomial issue. Two elderly patients developed myiasis after thoracic surgery, linked to operating room fly presence, which was resolved by environmental controls.

    Area of Science:

    • Medical Entomology
    • Infectious Diseases
    • Hospital Epidemiology

    Background:

    • Myiasis, infestation by fly larvae, is uncommon in hospital settings.
    • Nosocomial infections pose significant challenges to patient safety and healthcare management.

    Observation:

    • Two elderly patients developed hospital-acquired myiasis over three years.
    • Both patients underwent lengthy thoracic surgery in the same operating room during August.
    • Larvae identified as Phaenicia serricata were found in the nares of one patient and a chest incision of the other.

    Findings:

    • No tissue destruction or invasion was observed in either case.
    • Investigation identified contributing factors for fly presence in the operating room.
    • Implementing environmental controls, including sealing an access site and augmenting insecticide spraying, prevented further cases.

    Implications:

    • This case series highlights an unusual nosocomial problem requiring attention from infection control professionals.
    • Effective environmental management and fly control are crucial in preventing rare hospital-acquired infestations.
    • Awareness of potential environmental hazards is vital for maintaining sterile operating room conditions.
