    Summary

    This study confirms the existence of Acute lymphoblastic leukemia (ALL) with the hand mirror cell (HMC) variant. The HMC variant of ALL is a distinct morphologic subtype, not an artifact, and responds to standard treatment.

    Background:

    • Acute lymphoblastic leukemia (ALL) is a heterogeneous hematologic malignancy.
    • The hand mirror cell (HMC) variant of ALL is a rare morphologic subtype.
    • Understanding rare variants is crucial for comprehensive cancer diagnosis and treatment.

    Observation:

    • A case of ALL with HMC morphology was diagnosed in a 26-year-old male.
    • Leukemic blasts exhibited characteristic HMC appearance (66% of blasts).
    • Electron microscopy confirmed the HMC morphology and increased mitochondria in these cells.

    Findings:

    • Immunophenotyping and cytochemical studies indicated a non-T/non-B lymphoblastic origin.
    • Cytogenetic analysis revealed a 46XY karyotype.
    • The patient achieved complete remission with standard chemotherapy (vincristine, prednisone, L-asparaginase).

    Implications:

    • This case reinforces that the HMC variant of ALL is a genuine morphologic subtype.
    • The findings support the lymphoblastic origin of HMC leukemia.
    • The successful treatment response suggests standard ALL protocols are effective for this variant.
