    Background:

    • Chronic monocytic leukemia (CML) is a rare hematologic malignancy.
    • Distinguishing CML from other myelomonocytic disorders is crucial for accurate diagnosis and treatment.

    Purpose of the Study:

    • To analyze the clinical, hematologic, and histologic features of chronic monocytic leukemia.
    • To review and re-evaluate previously reported cases of CML in the literature.

    Main Methods:

    • Analysis of clinical, hematologic, and histologic data from five CML patients.
    • Comprehensive literature review of 28 reported CML cases.

    Main Results:

    • Patients presented with splenomegaly and initial anemia; monocytosis and leukocytosis developed post-splenectomy.
    • Only one of 28 literature cases met the strict criteria for CML, highlighting diagnostic challenges.
    • Survival post-monocytosis detection ranged from five to eight months.

    • Chronic monocytic leukemia is a distinct, albeit rare, hematologic entity with a characteristic clinical progression.
    • Splenectomy is not indicated for CML, unlike in hairy cell leukemia.
