    Summary

    Blastomycosis can affect immunosuppressed patients, but their symptoms resemble those in non-immunosuppressed individuals. Unlike other fungal infections, blastomycosis rarely occurs in patients with T-cell deficiencies.

    Background:

    • Blastomycosis is a fungal infection caused by *Blastomyces dermatitidis*.
    • Immunocompromised individuals are at higher risk for opportunistic fungal infections.
    • Distinct clinical syndromes are often observed in immunosuppressed patients with histoplasmosis or coccidioidomycosis.

    Observation:

    • This study reviewed 78 patients diagnosed with blastomycosis.
    • Six patients (7.7%) had received glucocorticoid therapy or had hematologic malignancies.
    • Clinical presentation in these patients mirrored that of non-immunosuppressed blastomycosis cases.

    Findings:

    • Blastomycosis in immunosuppressed patients does not typically present a distinct clinical syndrome.
    • Common presentations included chronic pulmonary infiltrates and isolated skin ulcers.
    • Early diagnosis correlated with a good therapeutic response.

    Implications:

    • The spectrum of underlying conditions in immunosuppressed blastomycosis patients differs from other endemic mycoses.
    • The rarity of blastomycosis in T-cell-deficient individuals warrants further investigation.
    • Understanding these differences can aid in diagnosing and managing blastomycosis in diverse patient populations.
