    Summary

    Acute Leptospira pomona infection can cause kidney damage through immune responses. Early detection is key, as immune-mediated kidney disease may be a transient phase in Leptospira infections.

    Background:

    • Leptospira pomona infection is a zoonotic disease that can affect multiple organs.
    • Renal involvement is a known complication of leptospirosis, but the underlying mechanisms are not fully understood.

    Purpose of the Study:

    • To investigate the renal pathology and potential immunologic mechanisms in patients with acute Leptospira pomona infection and renal insufficiency.

    Main Methods:

    • Renal function tests and kidney biopsies were performed on four patients.
    • Immunofluorescence and electron microscopy were used to examine kidney tissue.
    • Circulating immune complexes were assessed in one patient.

    Main Results:

    • All patients presented with acute interstitial nephritis.
    • Three patients showed no evidence of immune-mediated disease.
    • The fourth patient, biopsied later, exhibited glomerular immune deposits (C1q, C3) and circulating immune complexes, suggesting an immune-mediated process.

    Conclusions:

    • Renal lesions in Leptospira pomona infection can be immunologically mediated, particularly in later stages.
    • The timing of kidney biopsy is critical for detecting these transient immune-mediated changes.
