    Summary

    A child with acute lymphoblastic leukemia (ALL) and an 8;14 chromosome translocation had pre-B cells. This genetic abnormality and cell type suggest a poor prognosis for ALL patients.

    Background:

    • Acute lymphoblastic leukemia (ALL) is a cancer of the blood and bone marrow.
    • The 8;14 chromosome translocation is a known genetic abnormality associated with certain leukemias and lymphomas.
    • Pre-B-cell phenotype is an intermediate stage in B-cell development.

    Observation:

    • A case study of a child diagnosed with acute lymphoblastic leukemia (ALL).
    • The leukemic cells exhibited a pre-B-cell phenotype, characterized by intracytoplasmic mu-chain immunoglobulin (cIgM+) and the common-ALL antigen.
    • The patient's cells also showed an 8;14 chromosome translocation.

    Findings:

    • The 8;14 chromosome translocation, typically seen in Burkitt's lymphoma and B-cell ALL, was identified in a patient with pre-B-cell ALL.
    • This is the first reported case of an 8;14 translocation in pre-B-ALL.
    • The presence of the 14q+ chromosome abnormality in pre-B-cells suggests it may confer a proliferative advantage.

    Implications:

    • The 8;14 translocation and pre-B-cell phenotype in ALL may indicate a poor prognosis, similar to its association with B-cell ALL.
