    Background:

    • Chronic pain conditions, including postherpetic neuralgia, often present challenges for effective treatment.
    • Transcutaneous electrical stimulation (TENS) is a non-invasive modality explored for pain relief.
    • Previous treatment modalities had been unsatisfactory for a significant number of patients.

    Purpose of the Study:

    • To report on 7 years of clinical experience using transcutaneous electrical stimulation (TENS) for pain management.
    • To evaluate the long-term efficacy and patient adherence to TENS therapy in diverse chronic pain populations.

    Main Methods:

    • A cohort of 74 patients with postherpetic neuralgia and 161 patients with other chronic pain conditions were treated.
    • Patients were provided with TENS devices and various electrode types for home use.
    • Patient adherence and continued use of TENS were monitored over a 7-year period.

    Main Results:

    • Approximately 50% of patients discontinued TENS use within one month.
    • However, 25% of patients continued to use TENS for over two years, indicating sustained benefit for some.
    • No specific disease or pain type demonstrated significantly better or worse response to TENS; one-third of postherpetic neuralgia patients reported improvement from the start.

    Conclusions:

    • Transcutaneous electrical stimulation (TENS) can be a beneficial long-term treatment for selected patients with chronic pain, including postherpetic neuralgia.
    • Patient adherence varies, but sustained use suggests efficacy for a subset of individuals.
    • TENS represents a viable alternative for patients unresponsive to other pain management strategies.
