Summary
Transcutaneous electrical nerve stimulation (TENS) activates nerve fibers for pain suppression. However, this study suggests the peripheral physiological mechanisms of TENS may not significantly contribute to its analgesic effects.

- Electrical stimuli were used to induce pain on the finger, with TENS applied using various waveform types (rectangular, sine, triangular, hybrid).
- Microneurography assessed nerve fiber activation (A beta and A delta) and impulse conduction during TENS.

- TENS with tested waveforms primarily activated A beta and some A delta fibers.
- Pain suppression was most effective when TENS and noxious stimuli were applied to the same finger.
- Increased stimulus frequency led to variable latency and intermittent blocking of nerve signals, interpreted as changes in excitation threshold.
Conclusions:
- Peripheral nerve fiber activation by TENS does not appear to be the primary mechanism for pain suppression.
- The contribution of peripheral physiological mechanisms to the analgesic effects of TENS seems to be minimal.
