    Background:

    • Craniospinal irradiation is a critical treatment for pediatric central nervous system tumors.
    • Determining safe radiation doses is essential to minimize long-term side effects.

    Observation:

    • Three pediatric patients with medulloblastoma, anaplastic cerebellar tumor, or acute lymphocytic leukemia received craniospinal irradiation.
    • Estimated spinal cord radiation dose ranged from 1,100 to 1,200 rets.
    • All patients developed subacute transverse myelopathy after a 7-8 month latent period.

    Findings:

    • Myelopathy affected the cervical spinal cord in two patients and the thoracic in one.
    • Elevated spinal fluid protein levels were observed in two patients.
    • Myelography showed cord swelling in two patients; autopsy revealed necrotizing myelopathy in one.

    Implications:

    • Children may have a lower tolerance to spinal cord radiation compared to adults.
    • Radiation dose optimization is crucial for pediatric patients undergoing craniospinal irradiation.
    • Further research is needed to establish pediatric-specific radiation safety guidelines.
