    Summary

    Postoperative dizziness after stapedectomy is a rare complication. Histologic analysis of temporal bones revealed vestibular system pathologies in most affected patients, indicating diverse causes.

    Background:

    • Stapedectomy is a surgical procedure to improve hearing.
    • Long-term disabling dizziness is a rare but serious complication following stapedectomy.

    Observation:

    • A temporal bone collection of 117 specimens from stapedectomy patients was analyzed.
    • Nine patients (8%) reported severe long-term postoperative dizziness.

    Findings:

    • Histologic examination revealed peripheral vestibular system pathologies in 7 of 9 dizzy patients.
    • Pathologies included suppurative labyrinthitis, endolymphatic hydrops, fibrous adhesions, and a bone fragment near the saccule.
    • One case presented with an undiagnosed vestibular nerve schwannoma.

    Implications:

    • The diverse pathologies highlight multiple potential causes for post-stapedectomy dizziness.
    • A comprehensive neurotologic evaluation is crucial for accurate diagnosis and management.
    • Understanding these complications can improve patient outcomes after stapedectomy.
