    Summary

    Cyclophosphamide therapy selectively suppresses B cell function, reducing immunoglobulin secretion in patients with immune-mediated diseases. This targeted effect on B cells may explain its therapeutic efficacy.

    Background:

    • Nonneoplastic immune-mediated diseases involve complex immune dysregulation.
    • Cyclophosphamide is used to treat various autoimmune and immune-mediated conditions.

    Purpose of the Study:

    • To evaluate the effects of chronic low-dose cyclophosphamide on immune responses in patients with immune-mediated diseases.
    • To investigate the specific impact of cyclophosphamide on B cell and T cell functions.

    Main Methods:

    • Assessed immune cell function in 16 patients before and during cyclophosphamide therapy.
    • Measured immunoglobulin secretion and T cell blastogenic responses to mitogens (PHA, Con A, PWM).
    • Evaluated T cell helper function in T cell-dependent B cell assays.

    Main Results:

    • Cyclophosphamide selectively suppressed B cell function, particularly PWM-induced immunoglobulin secretion.
    • T cell function and helper capacity remained largely unaffected.
