    Summary

    High-density epidermoid cysts in the cerebellopontine angle show a unique bubblelike appearance on CT scans. This density is due to high protein content from a reaction to minor cyst fluid leakage.

    Background:

    • Epidermoid cysts are congenital tumors that account for a small percentage of all intracranial tumors.
    • Cerebellopontine angle epidermoid cysts can present with varied imaging characteristics.

    Observation:

    • A diffuse, bubblelike, high-density epidermoid cyst in the cerebellopontine angle was visualized using computed tomography (CT) scanning.
    • The high density was linked to a high protein concentration in the cyst fluid.

    Findings:

    • The high protein concentration resulted from a proliferative and exudative defense reaction to a minor leak of lipidic material from the cyst capsule.
    • Sudan black B stain confirmed the presence of lipidic material, supporting the leakage hypothesis.
    • This study presents the first morphological evidence of spontaneous cyst content leakage and is the second report of a dense intracranial epidermoid.

    Implications:

    • Understanding the imaging characteristics of dense epidermoid cysts aids in accurate diagnosis and surgical planning.
    • The findings highlight the dynamic nature of epidermoid cysts and their potential for spontaneous changes.
