Mallory-Weiss tears typically resolve with conservative care, but severe bleeding can occur. Intravenous vasopressin showed rapid, dramatic cessation of bleeding in three patients with torrential upper gastrointestinal bleeding.

    Area of Science:

    • Gastroenterology
    • Vascular Medicine

    Background:

    • Mallory-Weiss tears are a cause of upper gastrointestinal bleeding.
    • Conservative management is usually effective, but torrential bleeding requiring intervention is uncommon.

    Observation:

    • Five of 101 patients with upper gastrointestinal bleeding had Mallory-Weiss tears.
    • Three patients experienced torrential bleeding and were surgical candidates.

    Findings:

    • Two patients receiving systemic vasopressin pre-operatively experienced rapid cessation of bleeding.
    • Intravenous vasopressin demonstrated efficacy in controlling severe bleeding from Mallory-Weiss tears.

    Implications:

    • Systemic vasopressin may be a valuable therapeutic option for severe, persistent bleeding from Mallory-Weiss tears.
