    Summary

    This study investigated the link between HLA-B27 and sacroiliitis in Amish families. Sacroiliitis prevalence was similar in both HLA-B27 positive and negative members, suggesting other genetic factors are involved.

    Background:

    • The human leukocyte antigen B27 (HLA-B27) is strongly associated with ankylosing spondylitis and related conditions.
    • Sacroiliitis is a key feature of spondyloarthritis, and its genetic underpinnings are not fully understood.

    Purpose of the Study:

    • To clarify the relationship between HLA-B27 and sacroiliitis.
    • To investigate genetic factors influencing sacroiliitis in a genetically isolated population.

    Main Methods:

    • Evaluated 45 members from seven Old Order Amish families for sacroiliitis.
    • Focused on families with HLA-B27 positive and negative siblings, their spouses, and offspring.
    • Considered other HLA alleles, including Bw39 and B17, known to be associated with psoriatic arthritis.

    Main Results:

    • 24% of family members studied had sacroiliitis, regardless of HLA-B27 status.
    • The presence of HLA-B27 did not definitively correlate with sacroiliitis in this cohort.
    • Other HLA alleles (Bw39, B17) were present but did not fully explain disease distribution in B27-negative individuals.

    Conclusions:

    • The study suggests that factors beyond HLA-B27 influence sacroiliitis development in this Amish kindred.
    • Other non-linked genetic loci may be epistatic to HLA-B27.
    • Additional HLA alleles or population-specific factors may contribute to sacroiliitis prevalence in the Amish.
