    Background:

    • Paracoccidioidomycosis (Pcm) is a systemic fungal infection endemic to Latin America.
    • T and B cell mediated immunity play crucial roles in controlling Pcm.
    • Understanding immune dysfunction in Pcm is vital for effective management.

    Purpose of the Study:

    • To investigate T and B cell immunity in active and inactive Pcm.
    • To correlate immune parameters with disease status and IgE levels.
    • To explore the impact of T cell dysfunction on IgE production in Pcm.

    Main Methods:

    • Studied 20 Pcm patients (10 active, 10 inactive) and 8 healthy controls.
    • Assessed T cell mediated immunity, including peripheral E rosette-forming cells.
    • Quantified total IgE and P. brasiliensis-specific IgE antibodies in patient sera.

    Main Results:

    • Confirmed depression of T cell mediated immunity in active Pcm.
    • T cell dysfunction was not linked to reduced E rosette-forming cells.
    • Immunodepression reversed with chemotherapy.
    • Significantly increased total IgE levels in Pcm patients (active and inactive).
    • Low proportion of IgE against P. brasiliensis (0.4-0.6%).
    • Highest total IgE levels observed in active Pcm with immune depression.

    Conclusions:

    • Active Pcm is associated with significant T cell mediated immune depression.
    • Chemotherapy can reverse T cell dysfunction in Pcm.
    • Elevated total IgE in Pcm patients, particularly those with immune depression, suggests a role for T cell dysfunction in excessive IgE production.
