    Summary

    Captopril, an angiotensin-converting enzyme inhibitor, may cause nephrotic syndrome in some patients. This case highlights a potential adverse effect of this antihypertensive medication.

    Background:

    • Captopril is an angiotensin-converting enzyme (ACE) inhibitor used to treat hypertension.
    • ACE inhibitors are generally well-tolerated, but adverse effects can occur.

    Purpose of the Study:

    • To report a case of nephrotic syndrome potentially induced by captopril.
    • To highlight a rare adverse effect of captopril therapy.

    Main Methods:

    • Case report of a patient treated with captopril.
    • Clinical and laboratory evaluation including urinalysis, blood tests, and renal biopsy.

    Main Results:

    • Patient developed nephrotic syndrome (proteinuria 5-7 g/24 h, hypoalbuminemia, edema, hypercholesterolemia) while on captopril 450 mg/d.
    • Renal biopsy revealed subepithelial deposits on the basement membrane.

    Conclusions:

    • Captopril may be associated with the development of nephrotic syndrome.
    • Clinicians should be aware of this potential adverse effect when prescribing captopril.
