Renal isotransplantation without immunosuppression

R Weil, T E Starzl, K A Porter

    Annals of Surgery
    |July 1, 1980
    PubMed
    Summary

Kidney isografts functioned well for over 7 years without immunosuppression in four patients. This suggests prophylactic immunosuppression may not be necessary for kidney isograft recipients.

    Area of Science:

    • Nephrology
    • Transplantation Immunology
    • Surgical Innovation

    Background:

    • Renal transplantation is a critical treatment for end-stage renal disease.
    • Immunosuppression is standard practice to prevent graft rejection.
    • The necessity of immunosuppression in isograft recipients warrants further investigation.

    Purpose of the Study:

    • To evaluate the long-term function of renal isografts without immunosuppression.
    • To determine the appropriateness of prophylactic immunosuppression in renal isograft recipients.

    Main Methods:

    • Four renal isografts were performed.
    • Graft function was monitored over extended periods (7.5 to 17.7 years).
    • Patients did not receive prophylactic or therapeutic immunosuppression.

    Related Experiment Videos

    Main Results:

    • All four isografts demonstrated satisfactory function throughout the follow-up period.
    • One patient showed histologic evidence of recurrent glomerulonephritis at 7.5 years, with no impact on renal function or proteinuria.
    • No immunosuppression was administered to any of the recipients.

    Conclusions:

    • Long-term satisfactory function of renal isografts is achievable without immunosuppression.
    • Prophylactic immunosuppression may not be indicated for renal isograft recipients.
    • Further studies are needed to confirm these findings in larger cohorts.
