    Summary

    Individuals with Gardner syndrome have a significantly increased risk of developing periampullary carcinoma. Regular endoscopic screening for upper gastrointestinal disease is crucial for all patients with Gardner syndrome.

    Background:

    • Gardner syndrome is an autosomal dominant disorder characterized by colonic polyposis, osteomas, and soft-tissue tumors.
    • Periampullary malignancy is a rare but serious complication associated with Gardner syndrome.

    Observation:

    • A family with Gardner syndrome presented with multiple affected members developing periampullary malignancy.
    • Two to four family members showed evidence of periampullary cancer.

    Findings:

    • Individuals with Gardner syndrome have a 100- to 200-fold increased risk of periampullary carcinoma compared to the general population.
    • Susceptibility may be higher in certain subgroups, including those with duodenal involvement or all expressed Gardner syndrome characteristics.

    Implications:

    • All patients diagnosed with Gardner syndrome require periodic endoscopic surveillance for upper gastrointestinal malignancies.
    • Early detection through endoscopy can improve outcomes for patients at high risk for periampullary cancer.
