    Area of Science:

    • Gynecology
    • Endocrinology
    • Pharmacology

    Background:

    • Hormone replacement therapy (HRT) is used for menopausal symptoms.
    • Endometrial pathology is a concern with estrogen therapy.
    • Progestogens are used to mitigate endometrial risks.

    Purpose of the Study:

    • Evaluate symptomatic control with combined estrogen-progestogen therapy.
    • Assess bleeding pattern acceptability.
    • Determine the effect on endometrial pathology.

    Main Methods:

    • Single-blind pilot study with 25 patients over six months.
    • Used combined estradiol valerate with two progestogens: norgestrel and norethisterone acetate.
    • Administered therapy in cyclical and continuous regimens.

    Main Results:

    • Good symptomatic control across all groups.
    • Mild symptom recurrence during tablet-free intervals in cyclical groups.
    • Regular withdrawal bleeding with cyclical therapy; unscheduled bleeding with continuous therapy.
    • No endometrial hyperplasia observed post-treatment; two patients with prior hyperplasia showed normalized histology.

    Conclusions:

    • Combined estrogen and progestogen therapy is effective for symptom management.
    • Endometrial hyperplasia is unlikely with combined therapy.
    • Both progestogens were equally effective, and both regimens offered acceptable outcomes.
