    Stapled colorectal anastomoses demonstrated comparable reliability to hand sutures in low anterior resections. Stapling also allows for lower tumor resection, potentially improving surgical outcomes.

    Background:

    • Low anterior resection is a common procedure for rectal tumors.
    • Anastomotic technique (stapled vs. hand-sewn) can impact outcomes.

    Purpose of the Study:

    • To compare the performance of stapled end-to-end anastomoses versus hand-sewn sutures in low anterior colorectal resections.
    • To evaluate the safety and efficacy of stapled techniques in achieving lower anastomotic levels.

    Main Methods:

    • Retrospective analysis of 51 patients undergoing low anterior colorectal anastomosis.
    • Comparison of outcomes between 26 stapled anastomoses and 25 hand-sewn anastomoses.

    Main Results:

    • Stapled anastomoses: 2 cases (7.7%) of leakage/fistula, no reoperation needed.
    • Hand-sewn anastomoses: 2 cases (7.7%) of fistula, 2 cases (7.7%) of leakage/peritonitis.
    • Stapling allowed for resection of lower tumors and extended resection levels below the tumor.

    Conclusions:

    • Stapled colorectal anastomoses are as reliable as hand-sewn sutures.
    • Stapling devices facilitate lower tumor resection and extended resection margins in anterior resection surgery.
