Darier's disease: a partially immunodeficient state

B V Jegasothy, J M Humeniuk

    The Journal of Investigative Dermatology
    |February 1, 1981
    PubMed
    Summary

Patients with Darier's disease exhibit immune system deficiencies, specifically T-cell abnormalities, contributing to increased infection risk. This immune dysfunction appears intrinsic and not caused by the disease or its treatment.

    Area of Science:

    • Immunology
    • Dermatology
    • Genetics

    Background:

    • Darier's disease (Keratosis follicularis) is a genetic skin disorder linked to severe infections.
    • The underlying cause of increased susceptibility to infections in Darier's disease is not fully understood.

    Purpose of the Study:

    • To investigate potential immune system inadequacies in patients with Darier's disease.
    • To explore the role of T-cell function in the pathogenesis of Darier's disease.

    Main Methods:

    • Assessed immune responses using skin test antigens and in vitro lymphocyte stimulation.
    • Evaluated T-cell and B-cell counts and responses to mitogens like Con A, PHA, and PWM.
    • Tested the effect of patient serum and 13-Cis-retinoic acid on immune function.

    Main Results:

    • Most Darier's patients displayed anergy to skin antigens and impaired leukocyte inhibitory factor (LIF) production.
    • A unique lack of T-cell proliferation in response to Con A was observed in all patients, distinct from normal PHA and PWM responses.
    • Immune function was not suppressed by patient serum or improved by 13-Cis-retinoic acid treatment.

    Conclusions:

    • Patients with Darier's disease exhibit specific T-cell abnormalities, suggesting an intrinsic immune defect.
    • These T-cell alterations may serve as a marker for Darier's disease and contribute to infection susceptibility.
    • The immune dysfunction appears to be a primary feature of the disease, not secondary to infections or treatment.
