    Summary

    A young man developed a large ventricular septal defect (VSD) after a heart attack. Surgical repair of the VSD and left ventricular aneurysmectomy led to a full recovery and normal life.

    Background:

    • Ventricular septal defect (VSD) is a rare complication of myocardial infarction.
    • Congestive cardiac failure can be refractory to medical management in such cases.

    Observation:

    • A young male patient presented with a large VSD following extensive myocardial infarction.
    • The patient experienced medically refractory congestive cardiac failure.

    Findings:

    • Simultaneous surgical closure of the VSD and left ventricular aneurysmectomy was performed 6 weeks post-infarction.
    • The patient achieved a complete recovery and remained asymptomatic 14 months after surgery with minimal medical therapy.
    • The underlying cause was attributed to Prinzmetal's variant coronary spasm in a patient with normal coronary arteries.

    Implications:

    • Combined surgical repair of VSD and left ventricular aneurysmectomy is a viable treatment for post-myocardial infarction complications.
    • Early surgical intervention can lead to excellent long-term outcomes and improved quality of life.
    • Understanding the etiology, such as coronary spasm, is crucial for managing these complex cardiac conditions.
