Observation:
A heart transplant recipient experiencing rejection and heart failure was initially treated with dopamine as a vasopressor.
This treatment led to a significant decline in the patient's condition.
Findings:
Substitution of dopamine with dobutamine, a novel inotropic agent, resulted in improved cardiac function.
Dobutamine's direct cardiac action is suggested as the reason for its efficacy.
Implications:
Dobutamine may be the preferred agent for managing rejecting transplanted hearts.
It could also benefit patients with chronic heart failure by supporting cardiac output.
