    Summary

    Histologic evaluation of a bone graft 5 years post-implantation showed successful bone and cemental remodeling. Early re-entry of autogenous bone grafts appears safe for assessing periodontal regeneration.

    • Assessing the long-term histologic integrity of bone grafts is crucial for understanding regenerative outcomes.
    • Clinical success of a bone graft 5 years post-implantation prompted a detailed histologic examination.

    Observation:

    • A tooth requiring extraction due to labial root fracture was removed 5 years after bone grafting.
    • Histologic analysis of the extracted tooth and surrounding bone graft was performed.
    • The evaluation focused on the periodontal membrane apparatus and bone/cemental remodeling status.

    Findings:

    • Histology revealed a functional periodontal membrane apparatus with active bone and cemental remodeling.
    • Presence of necrotic bone spicules indicated residual graft material within a viable periodontium.
    • The graft material was integrated and supported periodontal regeneration.

    Implications:

    • Histologic findings support the clinical success of the bone graft.
    • Early re-entry for evaluation and contouring of autogenous grafts is justified.
    • No adverse effects on local periodontal regeneration were observed.
