Screening for toxoplasmosis in pregnancy

E J Broadbent, R Ross, R Hurley

    Journal of Clinical Pathology

    Summary

This study evaluated Toxoplasma gondii antibody prevalence in pregnant women using indirect haemagglutination antibody (IHA) and indirect fluorescent antibody (IFA) tests. The IHA test is suitable for screening pregnant women for toxoplasmosis.

    Area of Science:

    • Medical Microbiology
    • Immunology
    • Public Health

    Background:

    • Toxoplasma gondii infection during pregnancy poses risks to the fetus.
    • Accurate serological screening methods are crucial for identifying infected pregnant women.

    Purpose of the Study:

    • To evaluate the prevalence of antibodies against Toxoplasma gondii in pregnant women.
    • To compare the efficacy of indirect haemagglutination antibody (IHA) and indirect fluorescent antibody (IFA) tests for toxoplasmosis screening.
    • To explore potential risk factors associated with antibody prevalence.

    Main Methods:

    • Serological testing of 715 pregnant women using IHA and IFA tests.
    • Confirmation of positive sera using the dye test.
    • Questionnaires on diet and animal contact for a subset of the population.

    Main Results:

    • Prevalence of anti-Toxoplasma gondii antibodies was assessed, with 171 sera positive by IHA and 173 by IFA.
    • High correlation was observed between IHA and IFA test results, with 98 sera showing exact antibody concentration agreement.
    • The IHA test, available in kit form, demonstrated suitability for pregnancy screening.

    Conclusions:

    • The indirect haemagglutination antibody (IHA) test is a viable and commercially available screening tool for toxoplasmosis in pregnant women.
    • Antibody acquisition rates varied by age group, with higher rates in older women.
    • While animal contact was more frequent in older women, no conclusive evidence of recent or current infection was found in the study population.
