    Summary

    High insulin antibodies can stabilize blood glucose in diabetic patients. However, one patient with labile diabetes and insulin resistance stabilized after switching to porcine insulin, reducing antibody levels and improving glucose control.

    Background:

    • High insulin antibody titers are generally thought to buffer blood glucose fluctuations in diabetic patients.
    • Insulin resistance and highly labile diabetes present significant challenges in patient management.

    Observation:

    • A patient with severe diabetes lability and insulin resistance was treated with pure porcine insulin instead of mixed bovine/porcine insulin.
    • This switch aimed to reduce the patient's overall insulin requirement.

    Findings:

    • The switch to porcine insulin successfully reduced the patient's insulin dosage.
    • A significant decrease in insulin antibody concentrations was observed.
    • The reduction in insulin antibodies paralleled the stabilization of blood glucose levels and diminished diabetic lability.

    Implications:

    • This case suggests that high insulin antibody concentrations may contribute to poor glycemic control in some insulin-resistant patients.
    • Switching to a different insulin formulation could be a strategy to reduce antibody levels and improve diabetes management.
    • Further research into the role of insulin antibodies in diabetes lability is warranted.
